Admission Paperwork
We are delighted that you have chosen to send your child to Charlie Caterpillar's Day Nursery.
Please complete the following admission paperwork and send it to our secure email ready for your initial parent meeting. Our administrator will input your information onto our data base and register you for the parent zone app. During your initial parent meeting, you will get to know your key person and make preparations to settle your child in to nursery life.
We are really looking forward to meeting you.
Please complete Child Registration Form, Terms and Conditions, Privacy Policy and Parent Base Line for all children.
Your Key Person will complete a Health Care Plan with you at your parent meeting if your child has an allergy, medical condition, special educational need or disability.
We will also complete a Funding Confirmation Of Attendance Form and EYPP Registration Form if your child is in receipt of 2,3 or 4 year old funding. Printed copies will be available for when you meet with your Key Person.
